Primary Work Location: Eddie’s Place, BHBH, other sites as needed 

Eddie’s Place is a medically supported shelter for unhoused people in the East Bay who need medical stabilization. Eddie’s Place provides nursing, in-home care, shelter, meals, security, programming, and housing coordination. 

The BHBH program provides housing and supports for people navigating the Alameda County Behavioral Health Care Court. BHBH will provide shelter, caregiving supports, meals, security and more. 

 

Job Summary:  

Cardea Health is in the process of expanding its housing services and is seeking a Housing Director to join during this exciting phase of growth! The Director of Housing will serve as a leader in Cardea Health’s housing work and will partner closely with the Eddie’s Place Program Director to build and improve housing processes.  

The Director of Housing will supervise and manage the housing team and will develop workflows and systems to support housing resource linkage for individuals who have experienced chronic homelessness. The Director of Housing will be building a program for the provision of housing community support, including housing navigation and tenancy sustaining services. They will lead their housing coordination team to help individuals navigate the complex housing system, identify the appropriate resources they will qualify for, help them successfully exit Cardea Health programs in a timely manner, and sustain housing within the community if they are placed in permanent housing upon exit from the program.  

Cardea Health staff collaborate with other housing programs and social services to create a safe, independent living environment for a dynamic and vulnerable population. The ideal candidate will be invested in Cardea Health’s mission of serving marginalized populations with compassion and empathy. The Director of Housing will need to be flexible, patient, and willing to work in challenging environments with a diverse population. Experience with homeless or incarcerated populations is highly desirable. This is an excellent position for someone who is seeking to gain experience in a healthcare social service setting. 

Job Duties & Responsibilities: 


Leadership Duties: 

  • Manages and supervises a team of housing coordinators 

  • Leads the implementation of the housing community supports program and will manage the provision of housing community supports once implemented 

  • Develops a tracking system for following clients who have moved on from Cardea Health’s site-based programs, in order to provide ongoing housing navigation and tenancy sustaining services 

  • Develops a system for documentation for the housing coordinators and audits documentation 

  • Implements a system for HMIS data entry and auditing 

  • Creates and manages housing workflows and processes 

  • Manages caseload assignments for the housing coordinators 

  • Supports team in identifying appropriate discharge plans/locations for all clients 

  • Manages throughput and ensures clients are being discharged within their anticipated length of stay from Cardea Health programs 

  •  Manages collaboration between the housing and clinical teams 

  • Builds and maintains relations with community agencies to expand access to resources and increase institutional knowledge 

  • Works closely with the Eddie's Place Program Director to determine strategy for the housing work at Cardea Health 

Direct Practice Duties:  

  • Manages a caseload of shelter residents (this number will vary depending on the needs of the sites) 

  • Completes screenings and assessments to identify housing needs and factors contributing to housing instability, including functional, environmental, employment, educational, and health information, as appropriate. Develops housing plans for each resident and ensures they are on track to successfully exit the program by or on their anticipated discharge date 

  • Assists shelter residents on caseload to become document ready for future permanent housing options if appropriate 

  • Provides tenancy support for residents who are placed in permanent housing following their stay at one of our programs as indicated 

  • Helps connect residents to Housing Resource Centers (HRCs) and complete Coordinated Entry System (CES) assessments 

  • Facilitates shelter referrals 

  • Completes and maintains documentation of their work using the standard work developed by Cardea Health 

  • Develops and maintains collaborative relationships with providers, facilities, community resources, etc 

  • Serves as a strong advocate to help shelter residents obtain the housing resources they are eligible for 

  • Maintains confidentially in accordance with the Health Insurance Portability and Accountability Act (HIPAA) 

  • Practices harm reduction principles on site, including interventions that minimize harmful effects of substance use while treating all residents with dignity and respect 

  • Other duties and responsibilities as required 

Minimum Required Skills & Qualifications:  

  • Must be at least 18 years old 

  • High School Diploma or equivalent 

  • Must be authorized to live and work in the U.S 

  • Must have personal, reliable transportation to and from work sites and shifts 

  • Self-directed and able to work with minimal supervision 

  • Detailed oriented and highly organized 

  • Must provide proof of COVID vaccination or willing to be vaccinated 

  • Prior experience with homeless or incarcerated populations 

  • Proficiency in Microsoft Outlook, Word and Excel is required 

  • Applicants may be required to take a background check after a conditional offer is made Pursuant to State and local Fair Chance Ordinances, we will consider employing qualified applicants with arrest and conviction records 

Preferred Skills & Qualifications: 

  • Bachelor's or associate degree in a relevant field 

  • Previous experience with housing navigation and knowledge of the housing landscape in Alameda County 

  • Previous leadership experience.  

Physical Requirements:  

  • Frequent sitting or stationary work 

  •  May need to stoop, kneel, crouch, stand, walk, use arms, hands, and fingers.  

  • Will frequently need to communicate and listen.  

  • Will need access to reliable transportation to travel across sites. 

  • May be occasionally exposed to toxic or caustic chemicals. 

Work Environment:  

  • In-person onsite at a supportive housing site 

  • Travel between multiple programs in Alameda County
