Reports to: HR Manager
Position Summary
Receives, evaluates, and coordinates referral activity.
Educate client on services.
Distributes information to appropriate manager and HR manager for staffing and scheduling of employees.
Qualifications
- High School Diploma
- Associate preferred.
- Knowledge of home care industry, Medicaid waiver, and private payers.
- Excellent communication skills (verbal and written), interpersonal skills, and documentation skills.
- Demonstrated organizational and time management skills, critical thinking, and ability to work independently.
- Computer data entry skills.
Essential Functions/Areas of Accountability

Referrals
- Check email often for referrals.
- Contact client immediately when referral is given.
- Set-up time/date to meet client (try to meet clients quickly so other agency won’t get to them first).
- Assess the client (only take clients that you know we can staff)
- Call old referrals 1-2 months after getting the referral.

Intake
- Walk through the client’s home.
- Contact case manager or manager if client needs supplies, food, and/or equipment.
- Have client sign the agreement contract.
- Reach out to the client case manager or notify the office manager that the client has signed up.
- Reach out to HR manager to find staffing for the client

Summaries
- Write summary of the client that you signed up.

Documents
- Add new client on Alora plus.
- Scan documents in Alora plus.
- Complete Care plan.
- Update spreadsheet accordingly

Complaints
- Investigate complaints within 48 hours.
- Document complaints.
- Resolve the issue.

Community Outreach
-
Communicate with case managers.
- Issues with the client
- Updates on the client
- Request for more referral
- Request in increasing client hours.
- Attend job fairs.

Community service events.
- Bingo at facilities.
- Pancake bar, etc.
- Call and check on 1-2 clients daily.

Communicate with case managers.

Supervisory visits
- Visit 2-4 clients weekly.
- Do Google reviews with client and caregiver while at the client home.

Demonstrates teamwork and effective communication to accomplish Agency and client goals.
- Participates Care United Home Care Agency LLC meetings.
- Shares client observations and pertinent information.
- Maintains confidentiality in all aspects of the job.
- Performs job in compliance with Agency policies and procedures and professional and community standards.
- Performs other duties and activities as deemed necessary.
Work schedule:
- Works Monday – Friday from 8:00 am – 4:00pm.
- 30 minutes to 1 hour lunch
- Work On-Call Monday 4pm- Monday 8am (7 days) in rotation (may have to fill in for office staff that calls in sick). - Permanent employees only
