Position Overview

A Registered Physical Therapist (PT) administers physical therapy to patients on an intermittent basis in their place of residence. This is performed in accordance with physician orders and plan of care under the direction and supervision of the Director of Clinical Services.


Summary of Responsibilities

  • Provide physician orders and plan of care under the direction and supervision of the Director of Clinical Services.
  • Work in conjunction with all other home care personnel in patient care planning.
  • Direct and supervise Physical Therapy Assistant and Home Health Aide, as assigned.
  • Take initial patient history and make an initial evaluation.
  • Consult with physicians regarding changes in treatment.
  • Instruct patients and family in-home programs and activities of daily living.
  • Complete and submit OASIS assessments, reassessments, transfers, resumptions of care, discharges and significant change in condition.
  • Appropriately utilizes ICD-10 codes.

Requirements

  • Graduate from a physical therapist curriculum approved by the American Physical Therapy Association.
  • Currently licensed as a physical therapist in the state of Washington.
  • Two (2) years of experience as a physical therapist, is preferred.
  • Certified Home Health agency experience, preferred.
  • Effective and professional written and verbal communication skills.
  • Strong organizational and problem-solving skills.
  • Detail-oriented, self-motivated, and capable of efficient time management.
  • Critical thinker capable of working collaboratively with a team.
