NP Responsibilities

As a Nurse Practitioner, you will:

• Conduct comprehensive health assessments and develop care plans for geriatric residents
• Manage chronic and acute conditions under NP scope, including diagnoses and medication management
• Order and interpret labs and imaging, and coordinate referrals as needed
• Educate patients and families on health maintenance and disease prevention
• Participate in chronic care management and interdisciplinary quality-improvement initiatives
• Maintain accurate documentation using EMR (eCW experience preferred)

✅ NP Requirements

• Master's degree in Nursing and graduation from an accredited Nurse Practitioner program
• NY State nurse practitioner license (unrestricted)
• Active DEA registration to prescribe medications
• Ability to bill as an NP under Medicare/Medicaid
• Reliable transportation for travel across the specified counties

Preferred NP Qualifications

• At least 3 years of clinical experience as an NP, ideally in geriatrics or long-term care
• Experience with eClinicalWorks or similar EMR systems
• Excellent communicator, team player, and quality-driven clinician
