HOME CARE NURSE

Job Summary:

We are looking for a skilled Registered Nurse (RN) to provide patient-centered care in a home setting.

Full-time and part-time positions are available.

Key Responsibilities:

• Conduct in-home patient assessments and develop individualized care plans

• Administer medications, IV therapy, wound care, and other nursing interventions

• Monitor patient health status and adjust care plans as needed

• Collaborate with physicians, therapists, and family members to coordinate holistic care

• Educate patients and caregivers on medication use, chronic condition management, and preventative care

• Ensure compliance with home care protocols and state/federal health regulations

• Provide emotional support and advocacy for patients and families

Requirements:

• Education: Associate or Bachelor’s degree in Nursing from an accredited institution

• Licensure: Active, unrestricted Registered Nurse (RN) license in the state of practice

• Experience: At least 1 year of clinical experience preferred; home health experience is a plus

• Certifications: Current CPR/BLS certification

• Strong communication, time management, and clinical decision-making skills

• Reliable transportation and valid driver’s license required

Preferred Qualifications:

• Experience in home health, hospice, or community-based care

• Proficiency in using EMRs
