What are the responsibilities and job description for the Case Manager position at Caregiver Grove?

The Case Manager will manage cases for their assigned clients to ensure that each client receives services needed for overall well-being and in accordance with his/her individualized service plan (ISP).


RESPONSIBILITIES:


  • Utilize basic clinical interventions to support client efforts toward sobriety, coping with symptoms of depression and anxiety, safe lifestyle, healthy socialization, suicide prevention, occupational and academic success, individual and family restoration, and overall well-being.
  • Acquire knowledge and maintain current contact and referral information of community resources to address client needs for support of sobriety and mental stability, food, clothing, housing, financial assistance, inpatient referrals, medical care, and safety.
  • Assess client needs for community resources and coordinate linkage to ensure they maintain an effective connection.
  • Complete and submit progress notes and other documentation within 24 to 48 hours.
  • Adequate computer skills to effectively utilize electronic health records for daily record keeping.
  • A cooperative team approach to support coworkers’ efforts to address client and agency needs.
  • Self-motivated and flexible to manage time, schedule client visits, and strategize effectively to achieve productivity requirements and timelines.
  • Meet regularly with assigned clients to develop and implement their individualized program plans/goals.
  • Advocate for assigned clients to collaborate with their personal supports and outside entities to enhance relationships and demonstrate a role for them to learn self-advocacy.
  • Review case files regularly to ensure on-going audit-ready status.
  • Collaborate with referral sources, local agencies, and legal entities as needed to coordinate client care, facilitate referrals, achieve and maintain safety standards, promote goodwill, and expand opportunities for client resources and clinician training.
  • Acquire and maintain effective techniques for crisis de-escalation, as well as basic intervention for medical and mental health emergencies for use with clients in multiple settings.
  • Perform all job duties in a manner that is friendly, respectful, and aligned with the agency’s Mission, Values, Patient Care Philosophy and Standards of Ethical Conduct.
  • All other duties as assigned.


 QUALIFICATIONS:


  • High School Diploma or equivalent. Bachelor’s degree in human service field preferred. 
  • 2+ years of combined experience working with the developmental disabilities population and in a mental health environment - REQUIRED
  • Chemical Dependency/Social Work licensure, highly preferred.
  • CPR/First Aid and NCI/CPI Certifications required within 90 days of employment.
  • Must be certified within 90 days of starting the position on First Aid, CPR, and De-escalation Training.
  • Must maintain a current and valid Ohio or Michigan driver’s license, a good driving record, adequate insurance, and be willing to use a personal car to perform job duties.
  • Computer skills including Excel capabilities with the ability to create reports.
  • Strong organizational, time management, multi-tasking, and problem-solving skills.
  • Impeccable integrity, judgment, and professionalism.
  • Must be analytical and solutions-oriented.
  • Ability to work well in a high-growth, fast-paced environment.
