What are the responsibilities and job description for the Nurse Practitioner / Physicians Assistant position at Carespring?
We need a Nurse Practitioner to join our team at Carespring Healthcare Management in Loveland, OH. You'll play a vital role in providing comprehensive transitional care to patients in the Northern Kentucky, Dayton, and Cincinnati Area.
As a Nurse Practitioner, you'll evaluate patients before admission and provide medical oversight. You'll educate regarding medication and disease management as well as develop individualized care plans. You'll continue until patients are seen by their primary care providers, ensuring a smooth transition from our facility to ongoing care.
Schedule:
- Monday to Friday
- Approximately a 9-hour day with lunch
- Flexible hours, typically around 8:00 am – 5:00 pm
- Hours may vary based on daily needs
- On-call is not initially part of the program but could be implemented if necessary
You'll Be Responsible For:
- Collaborating clinically with Physicians, Nurse Practitioners, Physician Assistants, case managers, and other team members to understand patient needs during transitional care.
- Assessing potential candidates for transitional care management and determining the patients who best fit this care model.
- Conducting medication reconciliation and educating patients/caregivers regarding medication, care plans, and disease management in a dedicated office setting.
- Coordinating the transition of care and services by working with case managers, primary care providers, and other relevant parties.
- Maintaining a focus on process improvement to optimize the care model.
- Exhibiting leadership qualities and contribute to root cause analysis in and effort to prevent unnecessary hospitalizations and enhance clinical outcomes.
You'll Need To Have:
- A Nurse Practitioner License for Kentucky and/or Ohio
- The ability to work both independently and collaboratively
- Proficiency in teach-back methods (preferrable)
- Excellent organizational skills, adept at multitasking and prioritizing in a fast-paced environment
- Exceptional written and oral communication skills
- The ability to effectively interact and communicate with a diverse group of individuals, including patients, physicians, hospital staff, and colleagues
