What are the responsibilities and job description for the Case Manager position at Caring Companions At Home?
We are currently in need of a full-time case manager for an established home care company. This position would be ideal for a retired Gerontologist, RN, LVN or Social Worker.
Job Duties and Responsibilities
· Responsible for providing comprehensive case management to home care clients.
· Completes an initial Intake from the clients or family members needing care.
· Completes an initial assessment in the client’s home to determine the level of care and hours needed.
· Completes a thorough Plan of Care after assessing all the needs of the client.
· Communicates to the assigned caregivers the plan of care of the clients.
· Assists the client with the activities of daily living and facilitates the client’s efforts toward self-sufficiency and optional comfort care at home.
· Makes follow-up phone calls and visits regularly to see how the client and caregivers are doing.
Qualifications
· Prior experience is preferred but not necessary.
· Compassionate personality.
· Computer literate.
· Excellent communication and telephone skills.
