```Duties:```
- Provide in-home care and assistance to clients based on their individual needs and care plans
- Assist with activities of daily living, including bathing, dressing, grooming and toileting
- Assist with medication reminders
- Assist with meal preparation and feeding
- Perform light housekeeping tasks, such as, but not limited to; laundry, dishes, and tidying up
- Provide companionship and engage in meaningful activities with clients
- Monitor and report any changes in clients' condition to the appropriate healthcare professionals that may also include periodic caregiver conference updates
```Qualifications:```
- High school diploma or equivalent
- Previous experience in caregiving or a related field is preferred but not required
- Compassionate and caring nature
- Excellent communication and interpersonal skills
- Ability to follow instructions and work independently
- Reliable transportation to travel to clients' homes
- Ability to pass a background check and drug screening
