One Health Care Management (a division of Carobell, Inc.) provides whole integrated care management services to members and their guardians. Whole integrated care management is all about making sure that members' health and well being needs are being met in a timely and effective manner. Thius total care management service includes meeting with members and guardians, reviewing services, making recommendations and assisting members in receiving and utilizing properly the services they receive. We are currently seeking professionals with experience working in care/case management and/or social services.
Successful candidates must hold a bachelor’s degree in a social service field, have at least two year’s experience working directly with the MR/IID, mental health and/or substance use disorder population, be team oriented, have a valid drivers license and must pass a drug screen and criminal background check.
