Position Overview

We are seeking a dedicated Assertive Community Treatment Nurse (RN) to join our ACT team in Charlotte, NC. This role involves providing clinical services to individuals receiving ACT services, with a focus on medication monitoring, crisis intervention, symptom management, and education for clients and their families. As an integral part of the ACT team, the nurse will ensure that all medical and clinical aspects of care are delivered effectively to improve clients' overall well-being.

Location: Charlotte, NC 

Compensation: $72,666

Key Responsibilities

• Clinical Care:

  • Deliver psychopharmacologic and medical services within the scope of practice.
  • Provide symptom assessment, management, and individual counseling.
  • Support clients with substance abuse treatment, daily living skills, and social development.
  • Conduct crisis assessments and interventions as needed.

• Medication Management:

  • Monitor, document, and manage medications for all ACT clients.
  • Coordinate with healthcare providers regarding clients' physical healthcare needs.

• Service Coordination:

  • Collaborate with the ACT team to monitor each client’s clinical status and response to treatment.
  • Assist in identifying, planning, scheduling, and implementing services for clients.
  • Develop and implement transition plans when clients enter or exit services.

• Consultation and Collaboration:

  • Consult with primary care providers and other healthcare professionals to ensure comprehensive care.
  • Coordinate with state and community inpatient facilities when needed.

• Documentation:

  • Maintain accurate and timely documentation, including treatment plans, service notes, and activity records.

• On-Call Duties:

  • Participate in crisis phone and on-call rotations with other team members.

Qualifications

• Licensure: Must hold a current and valid RN license to practice in North Carolina, issued by the NC Board of Nursing.
• Experience: Experience in behavioral health or psychiatric nursing is preferred.
• Skills: Strong clinical, organizational, and communication skills; ability to work collaboratively in a multidisciplinary team.
