Registered Nurse Care Manager (RNCM):
Job Type: Full-time
Our clinic/day center operates Monday through Friday from 8:30am until 5:00pm and observes most major holidays.
ESSENTIAL FUNCTIONS:
· Care Management of Assigned Participant Panel
· Oversight of Care Transitions
· Plan of Care Development
· Participant/Caregiver Education Regarding Chronic Diseases
· Quality Improvement Initiatives
· Utilization Review and Analysis
EDUCATION AND EXPERIENCE:
- Must be a graduate of an accredited school of nursing with minimum of two years’ clinical nursing experience and one year’s experience caring for the frail and elderly. Current RN nursing license required.
- Would receive basic CPR and First Aid in orientation if they did not already possess certification.
- Candidates with a Bachelor of Science degree in Nursing preferred but not required. Prior experience working in home care or long term care setting, care management, or PACE are highly desirable.
Pay will be based upon skills and qualifications.
