Description
CTS Health Services is a community mental health services center that specializes in a holistic approach to health and wellness for children, parents, and adults of varying circumstances and mental challenges. We provide one-on-one behavioral interventions for our clients and their support circles to help our clients navigate through life within their community at their highest level.
The Psychosocial Rehabilitation Facilitator will facilitate regular PSR Services to ensure participants are provided with active treatment that will improve or maintain the individual's condition, improve functional level, and prevent institutionalization by teaching emotional, cognitive, and social skills. The These programs are aimed to help individuals live as independently as possible and become fully integrated into the communities in which they live. The Psychosocial Rehabilitation Facilitator will be consistently scheduled onsite for the provision of PSR Services per Rule 132.
Requirements
Educational requirements to be qualified as a Psychosocial Rehabilitation Facilitator:
- Master's Degree in a human service field (Social Work, Counseling, Human Services, Psychology, etc.),
- Behavioral Screening Certification (IM CANS), (CTS will supply resource to gain certification if not in possession upon hire)
- Bilingual (Spanish or Polish) (preferred)
Responsibilities of the Psychosocial Rehabilitation Facilitator:
- Maintain the minimum hourly billable caseload of 25 per week assigned.
- Facilitate PSR services and its curriculums.
- Work with PSR Director to update monthly calendars pertaining to PSR services.
- Guide clients, individually and in group sessions, to assist in overcoming dependencies, adjusting to life, and making changesby providing support in the development of skills and strategies for dealing with their problems.
- Prepare and maintain all required treatment records and reports.
- Collect information about clients through interviews, observation, and tests.
- Act as client advocates to coordinate required services or to resolve emergency problems in crisis situations.
- Implement program activities based on clinical experience and knowledge, utilizing co-occurring disorder treatment interventions, CBT, problem-solving and social skills, and update as required.
- Collaborate with U/M and staff members to ensure PSR service eligibility on IM CANS.
- Assess clients' physical or mental condition based on review of client information.
Job Training Requirements:
Trainings and certificates required to perform duties as a Mental Health Therapist (will be supplied by CTS if not in possession):
