Prism Health is the only nonprofit LGBTQIA health center providing comprehensive primary care, behavioral healthcare, and pharmacy services in the Pacific Northwest (Oregon, Washington, and Idaho). Since opening its doors in 2017, Prism Health has become a model for culturally specific care and now serves over 1,500 patients with a wide range of services, from day-to-day primary care and pharmacy needs to targeted services like gender-affirming care and specialized population health screenings. Prism a program apart of Cascade AIDS Project is excited to announce that we are hiring skilled and team-oriented Advanced Practice Provider. The Advanced Practice Provider (APP) II provides comprehensive medical care to patients at our integrated primary care and behavioral health clinics, which specialize in gender-affirming care, HIV care, and substance use disorder treatment. The APP works autonomously and in collaboration with various staff to diagnose and manage patients’ healthcare. The APP must be compassionate, self-confident, analytical, personable, and calm under pressure. This position reports to the Lead Provider and is responsible for clinical supervision of support staff, currently including Certified Medical Assistants.
This is a Full-Time role working at either of our clinic location on SE Belmont, NE Morris or float between the two. Travel throughout the county and evening and weekend work are occasionally required.
The starting pay is $146,687.65 a year. Which is equivalent to $70.52 an hour.
Who You Are
- You have Graduated from an accredited APRN or PA program
- You have an Oregon licensure from the Oregon Stated Board of Nursing (APRN) or Oregon Board of Medicine (PA)
- Your Certification: Member in good standing with the American Association of Nurse Practitioners (AANP) or American Nurses Credentialing Center (ANCC); or certified by the National Commission on Certification of Physician Assistants (NCCPA)
- You have Three (3) years in practice as a primary care provider
- You hold a Certification in CPR/BLS
- You have a strong understanding of issues particularly relevant to LGBTQ people.
- You have experience working with patients of all ages (pediatric to geriatric)
- You have sensitivity to, interest in, and competence in cultural differences, HIV/AIDS, minority health, sexual practices, chemical dependency, and a demonstrated competence in working with persons of color and gay/lesbian/bisexual/transgender community
What You’ll Do
- Provide full spectrum of primary care
- Provide all necessary documentation in the medical record (OCHIN Epic) of care provided to the patient
- Consult and collaborate with peer clinicians, Medical Director, and behavioral health colleagues
- Facilitate direct patient admissions into hospitals when needed
- Play key role on a multidisciplinary team to develop coordinated treatment plans
- Use primary care clinic protocols and procedures in providing services
- Anticipate and identify problems that affect patient care and use appropriate problem-solving processes to correct and eliminate problems
- Develop strong collaborative relationships with the LGBTQ & HIV medical community to enhance patient access to services, including specialists and consultant care
- Continually update own knowledge of LGBTQ health disparities through reading, conference attendance, lectures, and other educational opportunities
- Participate in quality efforts to improve health outcomes for patients
- Contribute to a positive work environment by utilizing trauma-informed principles in interactions with all patients, CAP employees, interns, etc., with an understanding, awareness, and respect for diversity
