What are the responsibilities and job description for the NHTD/TBI Service Coordinator position at Castle Rock Home Care?

Provide NHTD/TBI applicants and participants with service coordination and case management. Responsibilities may include:

  • develop and implement Initial Service Plans
  • coordinate between service providers and participant
  • monthly in-home visits
  • orchestrate and document team meetings
  • provide ongoing support
  • Renewal Service Plans


Requirements Include:

  • BA plus 3 years case management experience or an Associate's with 5 years case management experience
  • ability to travel to 5 boroughs (public transit or your own car)
  • bilingual English/ Spanish

Salary : $55,000 - $75,000
