CARE COORDINATOR / HEALTHY SMILES PROGRAM
Catholic Charities of the Diocese of Stockton is seeking a part time Bilingual Care Coordinator to provide support to families with oral health education, outreach, presentations and information offer maintenance support to engage families in continuity of dental care. Receive referrals and provide care coordination of children to Denti-Cal oral care providers in San Joaquin. Annually engage in overall client –based surveying of children for oral health needs by identifying existing Denti-Cal target populations. Convene at Care Coordination. Good communication skills, willing to speak in from of large audiences, work independently and occasional evenings and weekends. Must possess a valid driver’s license and insurance and willing to drive own car to events. To apply.
