What are the responsibilities and job description for the Resident Assess Coord position at Catholic Health?
The Resident Assessment Coordinator is an integral part of our team and is responsible to establish assessment reference dates for all new admissions and PPS assessments. Assessment Coordinator will complete MDS for all Medicare/PPS and HMO’s as required. Monitor patient eligibility of Medicare Part “A” and monitor until patient no longer fits criteria. An ideal candidate will have a thorough understanding of scheduling assessments appropriately and maintain a backup schedule for every resident and patient of the facility on their assigned caseload. Must have a comprehensive understanding of denial letters and process when appropriate. Work cohesively with the interdisciplinary team and notify them in a timely manner for completion of MDS. Have knowledge of PRI’s, and be able to monitor and record a full house or quarterly assessment period
QUALIFICATIONS: Must possess a current NY State license as a Registered Nurse, BSN preferred. Must have at least 3 years experience in Long Term Care or a combination of Long Term Care and Acute Care. Familiar with the MDS and Medicare/PPS. Knowledge of RUGS 4 and PRI scores. Must be knowledgeable of Federal, State and local DOH regulations as they relate to Long Term and Sub-Acute Care. Familiarity with Federal Compliance and HIPAA regulations. RAC certification desired, but NOT required.
