Staff Pharmacists/Inpatient fill orders for medications, monitor patient medication therapies, provide drug information, and supervise and direct support personnel. In the absence of the inpatient pharmacy manager, a staff pharmacist/inpatient may be required to assume the essential responsibilities and perform the duties of the inpatient pharmacy manager.
Roles and Responsibilities:
- Interprets medication orders and transcribes to computerized patient profiles accurately, maintaining complete patient medication profiles.
- Prepares and dispenses medications, including sterile IV preparations, using proper techniques.
- Issues controlled substances to patient care areas and maintains records as required by law.
- Monitors medication therapy regimens for interactions, allergies, appropriateness, and therapeutic duplication.
- Provides clinical consultation, recommends evidence-based medication regimens, and suggests cost-effective therapeutic alternatives.
- Attends ABC alert codes, assists with pharmacokinetic and anticoagulation consult services, and reports adverse drug events and medication errors accurately and timely
Required Skills and Experience:
- Education - Graduate of an ACPE-accredited school of pharmacy with either a BS pharmacy or PharmD required.
- Licensure - Currently licensed in the State of New York.
- Demonstrates flexible in assuming other appropriate responsibilities
Preferred Skills and Experience:
- Health-system pharmacy experience preferred, but not required.
Location and Travel Requirements:
- Onsite
- M-F, Sa/Su 7am-3:30pm, or 7:30am-4pm Sa/Su: 12-8pm (rotating)
