Essential Functions: - Supervise and manage the assigned Residential Program in coordination with the Director of Residential Services, to ensure that the standards established by all relevant regulatory bodies and policies established by CCARC, Inc. Board of Directors are met.
- Annually develop the Individual Plan – IP for each individual that identifies, evaluates and implements, with the assistance of the Interdisciplinary team and the person centered planning process, the supports needed by the individuals, including but not limited to, identification of medical and health needs, transportation needs, assistance with community resources and assist in determining that adequate financial/insurance benefits are maintained. Review the plan as needed with the individuals and the team making adjustments and/or changes as requested.
- Record changes, progress and status of the resident’s objectives for all reviews.
- Organize and participate in team meetings within the Residential Programs to include:
- individual (resident) meetings
- residential - staff meetings
- any other Interdisciplinary Team meetings
- Hire, train, supervise, provide support and direction to the Residential Program staff, evaluate and recommend appropriate action regarding staff performance in accordance with the policies established by the Board of Directors, to the Director of Residential Services and assure that staff successfully meet and maintain the requirements of all certifications and trainings as established by relevant regulatory bodies and CCARC.

Essential Functions Continued: - Maintain and keep current all records within the Residential Programs - in accordance with CCARC’s record keeping system to include but not limited to:
- Individual Plans and Reviews
- Individual’s - residents - records
- Daily log, medical log, recreation log

- Financial records, individuals - residents - records and Residential accounts
- Inventory
- Fire drills, vehicle evacuations, vehicle maintenance, mileage forms, monthly attendance forms
- Staff schedules, time sheets, time off requests, overtime
- Progressive discipline information
- Staff evaluation forms
- Annual Emergency Fact Forms
- Individual - resident - Incident reports, Employee Incident Reports
- Medication Omission and Error Report
- Annual medical, emergency and release forms and to audit the residents’ programs in Therap (ISP’s, health tracking, TLogs, etc.) to ensure that they being completed.
- Monitor and remain knowledgeable of the operating budget of the Residential Programs, including but not limited to food and house monies, residents personal spending, recreation monies, entitlement program spenddowns for the residents, reducing turnover of staff and managing overtime.
- Administer medications in accordance with the State/DDS regulations and oversee the ordering and recording of all residents medications.
- Maintain regular contact and communication with the Director of Residential Services regarding Residential Program, residents/staff issues, concerns and updates.
- Arrange for 24-hour on-call administrative availability in case of emergency.
- Attend all agency staff meetings or other meetings.
- Assist Director of Residential Services in the residential audits and licensing requirements
