What are the responsibilities and job description for the Travel Social Worker position at CEDAR PARK MED?
We are seeking a dedicated and compassionate Social Worker to provide coverage across multiple healthcare facilities as needed. The Social Worker will perform comprehensive psycho-social assessments, develop and implement care plans, and provide intervention, management, and support for at-risk patients and their families. This role involves working closely with the multidisciplinary team to address social determinants of health, discharge planning, and aftercare needs.
***This is a traveler position. Only candidates residing outside a 50-mile radius will be considered.***
Key Responsibilities
- Assess patient needs and develop care plans.
- Coordinate discharge planning and aftercare.
- Address social determinants of health.
- Provide emotional support and advocacy.
- Maintain documentation and perform other duties as needed
Qualifications
- Education: Master’s degree in Social Work (MSW) from an accredited institution.
- Licensure: LCSW or LMSW preferred, based on state requirements.
- Experience: 3 years in a hospital and/or healthcare setting.
