- Conduct comprehensive psychosocial assessments of individuals seeking services at CHEMED Health.
- Utilize clinical expertise to evaluate patients' needs, including mental health concerns, social determinants of health, and relevant support systems.
- Collaborate with interdisciplinary teams to gather pertinent information and develop individualized care plans.
- Provide brief interventions and crisis management as needed during the intake process.
- Serve as the primary point of contact for patients during the intake phase, offering empathy, support, and guidance.
- Utilize electronic health records (EHR) systems to accurately document assessment findings and treatment recommendations.
- Work closely with other LCSWs and mental health professionals to facilitate seamless transitions and referrals for ongoing care.
- Stay informed about community resources and referral networks to effectively connect patients with additional support services.
Qualifications:
- Master's degree in Social Work from an accredited institution.
- Current licensure as a Licensed Clinical Social Worker (LCSW) in the state of New Jersey.
- Minimum of 2 years of clinical experience in a healthcare or mental health setting
- Strong understanding of diagnostic criteria and evidence-based treatment modalities for mental health disorders.
- Excellent communication and interpersonal skills, with the ability to establish rapport and build trust with diverse populations.
