What are the responsibilities and job description for the Adult Case Manager position at Center for Mental Health?
Full-time, Non-exempt position, Helena
Duties: Include providing assistance to adults with severe disabling mental illness by assisting the client to obtain services, such as income support, transportation, housing, rehabilitation services, and prevocational and vocational services. Maintain caseload of assigned clients and meet productivity goals. Complete comprehensive strength-based assessment and reassessment every 90 days. Develop written treatment plan that coordinates with other services including family supports and outside community services. Develop crisis plan based on recommendation of the clinician. Provide outreach and follow-up for individuals who stop treatment. May develop budget plans with client. Bargaining unit position, must participate in the Union Pension fund.
Qualifications: Minimum qualifications are a bachelor’s degree in a human services field and at least one year of full-time experience serving people with severe disabling mental illnesses. Other education and experience may be considered. Travel required. Must have vehicle, valid driver’s license, and may transport clients.
