For more than one hundred years, the Center for Transforming Lives (CTL) has given participants and their families the tools they need to break the cycle of poverty. Each year, CTL works with over 3,000 people from more than 1,000 families in Tarrant County to provide comprehensive services no matter where they are on their journey from poverty to prosperity. CTL's services are trauma-informed, two-generational, and incorporate best practices rooted in nationally backed research to ensure women and children achieve long-term success.
Center for Transforming Lives has an immediate opening in our growing Housing Services program for a Rapid Rehousing Case Manager.

The Rapid Rehousing (RRH) Case Manager has the primary responsibility to provide intensive community-based case management and supportive services to individuals and families experiencing homelessness utilizing a trauma-informed, two-generation approach.
RESPONSIBILITIES
- Demonstrate a clear understanding of trauma and its impact on adults and children as evidenced by the implementation of trauma-informed care principles through the provision of services and decision-making.
- Maintain open lines of communication with women served, the Multidisciplinary Team (MDT). Actively participate in MDT Meetings in the review of program participant strengths, needs, service plan goals, and program extensions as requested. Interface with building guests.
- Plan for discharge upon program admission. Collaborate with the family and MDT to evaluate for discharge readiness on an ongoing basis. Develop post discharge permanent housing goals.
- Promote long-term housing stability and prevent returns to homelessness by cultivating and maintaining on-going relationships with property managers/landlords.
- Cultivate and maintain on-going working relationships with landlords, local housing authorities, and other pertinent community agencies; act as a liaison when needed.
- Orient families to program expectations and housing requirements as indicated in the lease agreement and Housing Assistance Payment Contract (HAP).
- Re-determine rental assistance eligibility in accordance with Agency policy and Continuum of Care (CoC) procedures.
- Coordinate community linkages around additional needed services.
EDUCATION AND EXPERIENCE
Bachelor’s degree in Social Work, Psychology, Sociology, or related field. Two years of experience working with families experiencing complex traumas such as homelessness, poverty, substance abuse, various forms of victimization and mental health issues. Experience in the area of social services and in-depth knowledge of available community resources is required.
KNOWLEDGE, SKILLS, ABILITIES
- Effective written and oral communication skills, including the ability to effectively articulate CTL’s mission, vision, and message.
- Excellent interpersonal skills and the ability to work well with all levels of management, staff, women and children, work independently with minimum supervision and maximum accountability.
- Demonstrate cultural competency through the ability to understand, communicate with and effectively interact with people of various cultural backgrounds.
- Attention to detail and the ability to prioritize and execute multiple projects under demanding deadlines.
- Knowledge of Windows operating system and Office products. Ability to learn and use any other software programs needed in the operations of CTL programs.
- Valid driver’s license.
