Job Purpose
The overall purpose of the Suicide Prevention Case Manager (SPCM) is to provide recovery-oriented services to eligible Veterans and family members at risk of suicide using a harm-reduction and client-centered approach. The SPCM will ensure the immediate safety of all individuals being assessed.

The overall purpose of the Suicide Prevention Case Manager (SPCM) is to provide recovery-oriented services to eligible Veterans and family members at risk of suicide using a harm-reduction and client-centered approach. The SPCM will ensure the immediate safety of all individuals being assessed.
Qualifications
- Minimum two years of experience in case management including needs assessment and service planning
- Bachelor’s degree in social services. Substitution of Associate degree with 4-5 years of experience in case management including needs assessment and service planning is acceptable
- Experience working with Veterans strongly preferred
- Experience delivering services to individuals with complex and intensive needs, including mental disorders and/or substance abuse, strongly preferred
- Resident of the proposed service area with broad familiarity with the region, its people, and its resources preferred
- Valid Wisconsin driver’s license in good standing and current/valid auto insurance
- Ensure the immediate safety of all individuals being assessed
- Administer the Columbia Suicide Severity Rating Scale to eligible Veterans and family members that may be at risk of suicide
- Administer the baseline mental health screening
- Develop a Wellness Recovery Action Plan (WRAP) safety plan (including gatekeeper training)
- Conduct a needs assessment and develop an Individual Service Plan (ISP) using a client-centered approach
- Monitor progress on the ISPs
- Refer, coordinate, and assist participants with scheduling/attending initial appointments with needed services and conduct follow-up to ensure that appointments are being kept
- Conduct one-on-one training with participants in areas such as:
- Suicide prevention
- Resource availability
- Participant rights
- Basic financial skills (budgeting)
- Daily living skills
- Conduct outreach with all known referral sources as well as develop new referral resources
- Other duties as assigned or requested
