Job Summary :
Under the supervision of the Clinical Director, works with the clinical staff in the assessment of client's needs. Resolves all client issues by escalating concerns to Clinical or Administrative team. Client retention is the primary focus and all efforts to ensure client has positive treatment experience is top priority. Assists management teams in meeting clients' mental and physical health needs through communication and available resources to ensure the clients' ongoing well-being. Provides necessary case management to all clients in treatment to guarantee that their current needs are met, and their discharge planning is in place for a smooth transition to aftercare services.
Essential Functions and Responsibilities :
- Works as part of a multidisciplinary team to provide quality care to clientele, ensuring all client needs are attended to.
- Establishes and maintains positive rapport with all clients in treatment.
- Works closely with Utilization Review regarding any insurance requirements.
- Handles all discharge planning & reaches out to aftercare facilities and / or therapists to schedule the first aftercare appointment alongside the client.
- Consistently present in the facility and readily available to speak to clients when needed.
- Becomes familiar with all clients' treatment goals and motivations.
- Uses established rapport to assist in ATA blocking alongside the Clinical Team.
- Conducts weekly case management sessions with each client and documents them same day in the EMR.
- Actively listens to clients' concerns and resolves issues when able, and if needed, reports them to the appropriate manager for a resolution. Issues include, but are not limited to accommodations, food concerns, group therapy concerns, and all other aspects of treatment.
- Encourages clients to make the most of their treatment experience by continually emphasizing the importance of rehabilitation and recovery.
- Consistently communicates and reports client needs or concerns to the Clinical Director.
- Adheres to after-hours ATA and emergency phone tree protocol for all client-related behavioral concerns including wanting to ATA.
- Conducts crises management / intervention of clients or as directed by Clinical Director.
- Schedules all 3rd party appointments for clients which include psychiatric evaluations, chiropractor appointments, massages, mani / pedi's, hairdresser appointments, etc.
- Schedules all medical appointments for clients and verifies benefits and coverage of appointments through client insurance
- Coordinate and facilitate support groups or psychoeducational workshops for clients and their families to address specific mental health issues or coping skills.
- Collaborate with community organizations and resources to develop referral networks for clients in need of additional services, such as housing assistance, financial aid, or vocational training.
- Participate in treatment team meetings and case conferences to discuss clients' progress and coordinate care plans with other staff members.
- Works with clients to complete FMLA, EDD / SDI, COBRA or any other necessary paperwork needed in order to obtain ongoing treatment or work.
- Carry out other duties / responsibilities as delegated.
