SUMMARY:

The person in this position is a registered professional with advanced training, functions as a collaborative member of the health care team in a primary care setting. Under an integrated primary care and behavioral health and the patient centered medical home model, this individual provides outpatient child and adult health psychiatric services to referred CenterPlace Health patients. Responsibilities include evaluating and re-evaluating medical and bio-psychosocial conditions; developing and implementing treatment plans, including prescribing psychotropic medications; and monitoring and evaluating treatment outcomes. Additional responsibilities include consultation and participation with an integrated primary care/behavioral health team and providing primary care team case consultation/training on psychiatric conditions and psychotropic medications. The quality of care will be in keeping with the standards of the appropriate national organization. This practitioner shall be responsible for maintaining current medical, diagnostic, and therapeutic records. High moral character, ethics, and conduct are compulsory. Support and mentoring of Advanced Practice Providers and Physician Assistants is an expectation.

ESSENTIAL FUNCTIONS:

PATIENT MANAGEMENT

  • Evaluating referred patients by interviewing them, their families, and other relevant persons; conducting physical examinations; observing behaviors; reviewing medical histories and related documents; selecting, administering, and interpreting psychological tests; ordering laboratory tests and evaluating results.
  • Developing treatment plans by determining the nature and extent of cognitive, emotional, developmental, social, and behavioral disorders; establishing treatment goals and methodologies.
  • Treating patients by utilizing psychotherapeutic methods and medications; discussing progress toward goals with patients; providing instructions; monitoring effects of medications; supervising staff provision of services.
  • Maintaining interdisciplinary treatment by reviewing treatment plans and progress; consulting and collaborating with primary care physicians, mental health and substance abuse therapists, nurses, and other health care providers.
  • Assuring quality and safe service for patients and staff by enforcing policies, procedures, standards, rules, HRSA requirements, and legal regulations; participating in utilization reviews; remaining available for emergency consultations.
  • Improving staff competence by providing training and discussions regarding the interrelationship between psychosocial and physiological problems, psychotropic medications, crisis intervention, and related issues.
  • Updating job knowledge by participating in continuing medical educational opportunities; reading professional publications; maintaining personal networks; participating in professional organizations.
  • Enhancing department and organization reputation by accepting ownership for accomplishing new and different requests; exploring opportunities to add value to job accomplishments.

PATIENT EDUCATION

  • Educates patients on all aspects of physical and mental health maintenance.
  • Provides short-term, individual, or family teaching, as appropriate.
  • Assists with development and implementation of patient educational materials.

ELECTRONIC HEALTH RECORD (EHR)

  • Demonstrates basic understanding of the EHR
  • Order entry and workflow
  • Workflow dashboard • Document and Task Management
  • Problem flowsheets • Troubleshooting documents in the clinical inbox
  • Accurately and efficiently enters documentation into the EHR.\
  • Maintains historical records by documenting symptoms, medications, and treatment events; writing summaries.
  • Ensures all EHR documentation is completed accurately and timely.
  • Cosigns charts, as required, in a timely fashion.
  • Ensures all charts are closed out in the EHR in a timely manner.

QI/QA

  • Actively participates in quality improvement projects.
  • Participates in ongoing peer review of medical records, committee assignments, and QA/QI activities.
  • Participates in and presents Continuing Medical Education activities.
  • Complies with Center policy and procedures promotes and complies with NCQA PCMH standards of care.
  • Assists in identification, evaluation and intervention of quality issues in the clinical setting.

LEADERSHIP

  • Maintains ethical and professional standards deemed appropriate by peers.
  • Supervises team activities.
  • Provides supervision, education and direct assistance when appropriate to Novice Physicians, Advanced Practice Providers and student providers, as reasonable, considering the provider’s training and capabilities.
  • Assists with management of patient flow to assure effective and efficient care is provided to all patients

OTHER

  • Appropriately applies patient evaluation and management processes to clinical situations.
  • Maintains use of universal precautions at all times.
  • Completes Daily/Day end activities as assigned.
  • Completes all required training and attends meetings as assigned.
  • Protects/observes patient confidentiality per policies and procedures.
  • Recognizes and responds appropriately to violent/abusive situations, bomb threats, fire and emergency situations.

Qualifications


MINIMUM QUALIFICATIONS: (EXPERIENCE/SKILLS/ABILITIES/EDUCATION/CERTIFICATIONS)

  • Graduation from an accredited Nurse Practitioner Certified Program or have Master of Science in Nursing degree with Certification as a Nurse Practitioner
  • Possession of a current Florida APRN License required
  • A minimum of 3 years’ experience in a outpatient Psychiatric setting required
  • Effective oral and written communication and interpersonal skills required
  • Proficient with personal computers including Windows, Excel, Word, PowerPoint required
  • Experience working within an Electronic Health Record system (Athena preferred)
  • Board Certified required
