What are the responsibilities and job description for the Nurse Care Manager position at Centerpoint Health?
Centerpoint Health is seeking a Nurse Care Manager to work out of our Middletown and Franklin clinic locations. The Nurse Care Manager is a key support for patients as part of the Patient-Centered Medical Home (PCMH) during the continuum of care, either a portion or in its entirety, who assists patients in a standard of care using evidence-based disease state management clinical pathways. Though the emphasis will be on primary disease prevention and improving quality of care for chronically ill patients, the program will include outreach to patients to be more engaged in their overall health and wellness.
Job Duties:
Collaborate with hospital discharge teams, community care partners, and other providers to ensure smooth transition to the most suitable level of care.
Collaborate with healthcare team members to identify appropriate utilization of resources. Collaborates with Physician or other Licensed Independent Practitioner to arrange for further medical treatment, when needed.
Promote patient education and self-care management in order to reduce hospital readmission.
Promote current evidence-based practices; periodically reviews updates with clinical staff.
Provide phone triage for any patients for follow-up discharge.
Maintain a clean and safe environment. Follows infection control procedures.
Maintain patient privacy and confidential patient information.
Participates in follow up phone calls and follow up patient visits.
Uses nursing skills to evaluate patient care needs, prioritize treatment, and maintain patient flow.
Conducts a nursing assessment by taking a patient history and physical assessment and by obtaining vital signs and other medical tests. Provides treatment within scope of practice as defined by state law.
Provide consultation and support to the development, implementation and operation of a Quality Improvement Plan.
Promote current evidence-based practices, with periodic review and updates with clinical staff.
Evaluates health education/promotion needs of patients and provides necessary training and instruction.
Identifies emergency situations and provides care within legal scope of practice.
Other duties as assigned.
Certifications/Credentials:
Education: Associate’s or bachelor’s degree in nursing – LPN or RN
Certification and Licensure: Must maintain unrestricted RN/LPN licensure.
Experience: Prefer three years as a nurse, one year in care management, one year with Quality and Risk, hospital experience, and to have worked with PCMH and/or Joint Commission accreditation (PCMH).
Additional Skills/Qualifications:
Experience with patient-centered medical homes (PCMH) and community health centers.
Demonstrates knowledge of growth and development, assesses data reflective of the patient's status, and identifies each patient's requirements relative to age specific needs along the continuum of care.
Able to work collaboratively with clinical staff of all levels, assisting in optimizing delivery of patient services including knowledge of scope of practice of clinical team.
Knowledge of utilization management and quality improvement skills to deliver quality, patient-focused, comprehensive healthcare services to diverse populations.
Population management and knowledge of corporate compliance and risk management.
Attention to detail, ability to follow directions, and ability to think independently as appropriate.
Language: Bilingual in Spanish and English (Preferred)
