Centerpoint Health is an FQHC seeking a Physician for our community health center. We have locations in Franklin, Middletown, and Norwood, OH. We provide services in primary health care, dental, ob-gyn, and behavioral health, and are looking for a CMO to join our amazing team!
Description of Position/Purpose:
To provide direct primary health care services, with an emphasis on prevention and treatment of disease.
Job Duties:
Provide quality primary care to patients within scope of health center practice and discipline. Send patient referrals as required for patient care outside the scope of the agency or discipline for management of patient care.
Order, perform, and document patient care and treatment plan in the electronic medical record (EMR) to reflect assessment services rendered, history and physical, relevant diagnostics, treatment plans, and referrals. Documentation in the EMR should include quality performance measures as required, i.e., UDS, PCMH, and other quality performance measures as determined by the Quality Team.
Review treatment plans with patients, parents, and staff clearly and effectively. Obtain informed consent as needed when providing services with risk to patients.
Participate in administrative duties, including lab reports, charting, and medication refill authorizations.
Participate in resolution of patient grievances regarding clinic operations when designated by CMO.
Use time management skills to achieve productivity, as established by health center management.
Contribute to the health center goals with respect to quality improvement plans as designated by health center management.
Participate in meetings as requested, such as Staff and Provider Meetings, conferences, and other training,
Complete quarterly peer reviews of other Centerpoint Health providers.
Maintain continuing medical education (CME) requirements as required by Ohio law.
Formally collaborate with Centerpoint Health nurse practitioners and physician assistants and shall be available for consultations with nurse practitioners and physician assistants as required under Ohio law.
Comply with all applicable federal, state, and local laws to include the protection of personal health information as directed by the Health Insurance Portability and Accountability Act (HIPAA).
Participate in proportional evening hours and on-call services based on the needs of the health center.
Physician shall share equally in vacation and personal/sick days coverage of all physicians.
Participate in EHR development as requested by health care management staff.
Collaborate with the CMO or designee regarding the schedule.
Other duties, as assigned by the Chief Medical Officer.

Certifications/Credentials:
Education: MD or DO from an accredited Medical School required.
Certification and Licensure: A valid, unrestricted license to practice medicine in Ohio, with no pending or previous disciplinary action. A specialization in Family Practice or Family Medicine or Internal Medicine and/or pediatrics is strongly preferred. Current Active DEA registration required.
Experience: Two years’ experience as a physician performing outpatient care is preferred.
Additional Skills/Qualifications:
A competency in evaluating and treating outpatient primary care based on discipline.
Ability to perform the full range of preventive, acute, and chronic primary care and disease management.
Ability to work collaboratively within an integrated health care environment.
Demonstrates strong oral and written communication skills when interacting with patients, families, and other health care providers.
Ability to work with interruptions, manage priorities, and work unsupervised.
Ability to be adaptive, flexible, and responsive to the needs of the health center, patients, and families.
Proficient computer skills, including Microsoft Office. Knowledge of Electronic Health Records required.
Ability to spend significant time standing. Ability to use equipment required for the delivery of services.
Prior experience in collaborating with mid-level practitioners preferred.
