The RN Coordinator of Care (COC), responsible for organizing and directing home care services to patients. Ensures appropriate coordination among all professional and paraprofessional services related to the care provided to the patient.
Responsibilities :
- Completes an initial assessment of the patient and family to determine home care needs.
- Completes a comprehensive physical assessment of the patient.
- Assess home safety and makes suggestions when hazards are noted.
- Arranges for services / supplies ordered and manages the coordination of other professional
