Job Description: Assisting mental health consumers in building personal strengths, develop coping strategies and skills so they can build a supportive environment allowing them to function as independently as possible. Providing services in a variety of settings including group, classroom and one on one setting. The majority are delivered in a group or classroom setting. Essential Duties and Responsibilities: Plan with consumer for needed services, including identification of services and means of accessing those services. Initial service plan completed within 30 days of case management enrollment with measurable goals and objectives. Required documentation is completed according to standards and agency policy and procedures. Progress notes contain documentation of the discussion of progress toward or lack of progress toward service plan and/or treatment plan goals. Demonstrate good clinical judgment in decision making regarding consumers. Demonstrate ability to relate to consumer in an appropriate manner. Facilitate psycho-educational and activity groups as part of the Adult Day Treatment Program. Assist other staff in the supervision of consumers participating in the Adult Day Treatment Program. Other duties as assigned by Centerstone management staff. Meet program requirements for cases passing edit monthly. Is an active and vital member of the treatment team. Job Description Bachelor's degree in social work, counseling, psychology, or related field. Must have reliable transportation, liability insurance, and TN driver's license.
