What are the responsibilities and job description for the Wellness Coach position at Centerstone?
SUMMARY OF POSITION
Provides support services, information, education and support for clients. Promotes wellness, recovery and skill building to sustain better health. Provides prevention training for managing stress and problems, making healthy life style choices, recognizing and responding to relapse, managing crisis and achieving and sustaining recovery. Assists clients with personal recovery goals, assisting them in developing a natural support systems, and coaches clients to advocate for themselves within their healthcare systems.

ESSENTIAL DUTIES & RESPONSIBILITIES
- Establishes relationships and practices core coaching skills to transform client values/desires into action and maintain lasting change over time.
- Provides group and/or individual training/interventions focusing on improvement of the client’s functioning and health-related issues (e.g., symptom/medication management, life skills, healthy eating, weight management, smoking cessation, recognizing the dangers of alcohol and other drugs, community reintegration, etc.).
- Provides wellness consultation. Assists clients with developing their Wellness Recovery Action Plan (WRAP). Works in consultation/collaboration with clients, family members and other providers to implement plan.
- Empowers clients to self-manage chronic health conditions and solidify wellness goals. Assists clients with finding solutions utilizing motivational interviewing and finding motivation to complete their individualized recovery plan to achieve wellness goals. Assists clients in overcoming any barriers to goals, including access to care. Assists with navigating health care systems.
- Links clients to community-based social support/services and develop collaborative relationships with other departments, services, and community health care agencies to facilitate and support wellness and quality care.
- Provides ongoing case management services for assigned service recipients, including but not limited to assessment, planning, linking, monitoring, referral and advocacy.
- Provides transportation assistance to help clients achieve and maintain wellness goals and initiatives.
- Conducts intake for recipient entering services, including explaining services, gathering information needed on intake materials, ensuring service recipient understands all forms, assuring signature on appropriate contracts and consents, and submitting materials for processing, as assigned. Conducts assessment and write assessment summaries, as assigned.
- Assists service recipient in making discharge plans and provide follow up services as assigned.
Education Level
Participation in own personal recovery process in order to care for self. Must have the ability to share their recovery journey.
Years of Experience
Experience providing mental health services preferred.
Required Certification/Licensure
Ability and willingness to obtain credentialing if required.
