What are the responsibilities and job description for the Blended Case Manager position at Centerville Clinics?
Centerville Clinics is looking for a Blended Case Manager to join our Behavioral Health Base Service Unit. Bachelor's level applicants are encourage to apply, and/or individuals with valid RN license in the state of PA.
This position is of direct service nature to persons with refractory mental illness or who are at risk of serious mental illness.

Duties Include:
- Psychosocial assessment evaluation of consumer's strengths and weaknesses
- Developing of service plans with emphasis upon community integration, assisting the consumer to gain assess to resources and required services
- Developing and involving consumer support networks
- Providing life support and problem resolution services
- Serve on various committees and participate in agency administrative functions
- Complete requisite paper work requirements from Agency , county, and state that require thorough record keeping
Job Types: Full-time, Part-time
Pay: $19.44 - $20.32 per hour
