MENTAL HEALTH CASE MANAGER
Full time position for Mental Health Case Manager to assist with adult clients with severe mental health issues.
Duties: Provide direct case management services to individuals suffering from mental illness. Provide services such as: symptom monitoring, treatment/service planning, assistance in daily living, supportive services, crisis intervention, linkage/referral/advocacy, and support of client’s self-determination. Work in a supportive team environment as well as collaborating with a multidisciplinary clinical treatment team. Meet annual service delivery expectations. Provide transportation of clients in personal vehicle, as necessary. Participate in 24-hour emergency on-call system.
Due to COVID-19 – procedures and PPE supplies are in place, but applicants must be willing to work in the community with clients. Strong Interpersonal skills and ability to provide support in person and by telehealth.
Qualifications: Bachelor’s degree preferred. Candidates not possessing a bachelor’s degree but with relevant experience will be considered. Transportation and good driving record required as well as personal vehicle insurance for bodily injury/property damage liability.
Job Type: Full-time
Pay: $18.00 - $18.50 per hour
Expected hours: 40 per week
