Overview:
A full-time position providing both clinical supervision, as well as direct care including case management and assessment services for Central Connection clients. Central Connection is a unique mode of service delivery as our clients are transitioned to ongoing providers typically within 90 days. Clients are identified through an assessment process, and they may be referred to us from the community, hospitals, and forensic systems. We are currently in need of an individual who would like to continue to engage with clients face to face by completing assessments on an ad hoc basis, providing case management in the community, and mentor and provide supervision to a small team of assessors and case managers. Caseload size will range based on current needs but is typically in the 10-12 range. Goals including engagement and interactions that are outside the scope of the traditional "productivity" model. Many of our clients are "frequent faces" to our community and require a high level of engagement.
Duties:
- Provide clinical oversight of dependently licensed clinicians
- Collaborate with clients and families to identify available resources for housing, benefits, and other entitements
- Provide crisis intervention, advocacy, medication monitoring, transportation assistance, ADL assistance
- Work frequently with Hamilton County Probate Court to determine need of services and supplying creative problem solving
- Have frequent collaboration with professionals from multiple systems and other agencies to address treatment needs at both the client and programmatic level
Minimum Qualifications:
- Experience with working with severe and persistent mental health and/or persons with mental health and substance use needs
- Master's Degree in social work, counseling or marriage and family therapy
- Independent license in social work, counseling or marriage and family therapy
- Must be willing to obtain supervisory designation within 6 months of hire if candidate does not currently have the supervisory designation
- Experience providing direct care in the form of case management and assessment
- Effective time management and communication skills
- Ability to interact professionally with persons from diverse backgrounds
- Knowledgeable about the Hamilton County public behavioral health systems and other community resources
- Reliable transportation, a valid driver's license and current auto insurance for bodily injury/property damage liablity
