Central Coast Home Health is a fast-growing and locally owned organization with an excellent reputation throughout the San Luis Obispo and Santa Barbara counties. We are constantly growing and now seeking full-time Case Manager RNs to join our hospice team in Norther San Luis Obispo (Atascadero/Templeton/Paso Robles)
Our nurses deliver high quality care to our hospice patients in their own home environment, collaborating with an interdisciplinary team to promote patient and family choice, assure continuity of care, and enhance the quality of life of dying patients. This highly rewarding field requires organization and the ability to work independently, managing your own caseload and making your own work schedule.
Full time with a weekend rotation. NO Night Call!
Must have current CA RN license, CPR certification (AHA), 1 years of professional nursing work experience, reliable transportation, auto insurance, & good driving record. Hospice experience preferred but required. Must have excellent communication and interpersonal skills as well as strong clinical background.
