Non-negotiable salary (with licensure): $68,406.00 annually.

Responsibilities

The Central Community Health Partnership is a collaboration of five premier human service organizations in Central Massachusetts that have been selected to provide Behavioral Health Community Partner services and Long-term Services and Supports Community Partner services through the MassHealth program. For people with Long-term Services and Support needs, such as developmental disabilities, finding the right combination of services and knowing how to access them can seem to be an overwhelming task. Central Community Health Partnership stands ready to help MassHealth members with complex needs navigate the confusing array of choices. Seeking a full time (37.5 hours/week) Clinical Care Manager – Long Term Services and Supports for Elder Services of Worcester Area, Inc.’s contract with Central Community Health Partnership (CCHP) agency. The Clinical Care Manager will provide and oversee integrated care coordination and care management to individuals enrolled in the CCHP’s Long Term Services and Supports (LTSS) Community Partners Program. The Clinical Care Manager will supervise a team of Care Coordinators who provide integrated care management in collaboration with individuals served, community partners, medical and behavioral health providers. The Clinical Care Manager will also participate in trainings, meetings, statistics, program procedure development, and other tasks as listed in the job description.

Qualifications

  • LTSS Clinical Care Manager(s) shall have at minimum: At least two years of relevant professional experience and hold the following qualifications: A licensed registered nurse (RN), A licensed independent clinical social worker (LICSW), A licensed mental health counselor (LMHC), A licensed marriage and family therapist (LMFT), A licensed alcohol and drug counselor I (LADC I), A licensed psychologist; A licensed certified social worker (LCSW); A licensed rehabilitation counselor; or A registered occupational therapist. Certified Care Management License preferred, but not required.
  • A minimum of one year of Supervisory experience is preferred.
  • Computer proficiency, including experience with Microsoft products (Excel, SharePoint, Word) required.
  • Experience working with older adults, persons with disabilities, and knowledge of human services resources are preferred.
  • Knowledge and skills to work successfully in an interdisciplinary team are required.
  • Use of a reliable vehicle including active vehicle insurance with a current license to drive required.
