Consider joining our team as the Personal/Family Health Supervisor for the Isabella County Branch office of the Central Michigan Health Department to improve the health outcomes of the community. This salary position is highly regarded as a leader in the workplace and community who will guide and motivate their public health team. This individual must have excellent communication skills to accommodate health related needs and concerns for individuals in our communities while being detail oriented with exceptional technical skills working with electronic health record (EHR) systems. An emphasis for this position will be on Communicable Disease within the community and health department. In addition, the position will provide backup support to the specialized programs within the Personal Health Services Division. Other duties may include supervision of program staff, oversight of the daily operations, participate in program/clinic management, and administration, implementation, and evaluation of CMDHD health programs. Experience in outreach, promotion, program development and surveillance of public health programs, policies, procedures, and protocols is required. This position may include clinic responsibilities and is responsible for following the goals, objectives, policies, procedures, and standards of the CMDHD and appropriate State and Federal agencies. A minimum of three (3) years of experience in public health nursing or five (5) years of related supervisory experience required. BSN or equivalent recommended. This full-time position offers full benefits, including health/life/dental insurances, 20 vacation and 12 sick days with an additional 3½ paid days of personal time per year, 15 paid holidays, MERS defined contribution retirement plan with up to 10% employer contribution, and flexible scheduling are included.

General Summary: Under general direction of the Personal Health Director, the supervisor is responsible for organizing and leading delivery of care in clinics, homes and/or community settings for assigned program of public health services. Ensures program requirements are met, and monitors staff and agency compliance with standards, policies, and best practices. Provides orientation, consultation and guidance to program staff. Provides input on staff evaluations. Collaborates with multidisciplinary and multi-agency teams to achieve desirable outcomes for families and communities.

Essential Duties:

1.Supervises all personnel in the assigned.

- reviews nursing daily activity reports

- signs and approves pay vouchers, leave requests, comp time, and mileage vouchers

2. Coordinates daily staff assignments, including leave time and substitution.

3. Coordinates and participates in the activities of Personal Health Services in the branch office and with other divisions of the CMDHD.

4. Participates in the direct provision of health services in a clinical setting to individuals or families.

5. Participates in the development, implementation and evaluation of the department’s programs, policies, and procedures, making recommendations as appropriate.

7. Performs various personnel related functions, participates in the hiring, orientation, disciplinary actions, evaluation and ongoing education of staff.

8. Provides for agency representation in community.

9. Informs the Personal Health Services Director of facility, staffing, equipment, supply, staff development needs, and personnel issues.

10. Serves as a liaison between the staff and the Personal Health Services Director or other administrative staff as appropriate.

11. Reviews monthly program reports, collects data for annual report, collects information as requested for special grants/funding sources.

12. Keeps current on new practices, methods, and procedures in the public health field by reading articles, participating in staff in-services, consulting with MDPH consultants and attending workshops.

13. Meets with branch office staff, program staff, committees, supervisory staff, administrative staff and the Personal Health Services Director to further the operation of CMDHD.

14. Performs other duties as delegated.

Other Duties & Responsibilities:

  1. Performs other duties and responsibilities as necessary in response to emergencies, workloads or staffing shortages. Works flexible hours as needed.
  2. Participates in Emergency Preparedness Incident Command System (ICS) as assigned.
  3. Supports other program and office functions as necessary to ensure effective customer and community service.
  4. Participates in efforts to develop and promote program and agency services in conjunction with marketing team
  5. Maintains up-to-date vaccines including flu (influenza,) in accordance with CDC guidelines: Recommended Vaccines for Healthcare Workers, in which you allow CMDHD to access your current immunization status based on Michigan Improvement Care Registry (MCIR).

Minimum Qualifications:

  1. Bachelor’s degree in public health, nursing, social work or other human services field and advanced knowledge of theories, practices, and standards related to assigned program.
  2. Advanced knowledge of public health nursing, and assigned program requirements and practices, at a level normally acquired through three to five years of related, professional experience.
  3. Current Michigan Nursing license to practice as in their respective field in their field of study.
  4. CPR certification required.
  5. Analytical abilities necessary to evaluate and monitor program compliance and devise appropriate plans and strategies to achieve desired outcomes for individual, population and community health.
  6. Ability to plan and organize own work and accomplish multiple tasks in accordance with established practice and professional standards.
  7. Cultural sensitivity and ability to work with diverse communities and families.
  8. Communication and writing skills necessary to provide guidance and instruction to staff and collaborate with internal and external contacts in developing and implementing program plans, policies and procedures.
  9. Leadership and mentoring skills necessary to guide and support professional development of others.
  10. Reliable transportation with valid Michigan driver’s license and certificate of insurance to travel as needed to other locations for work, training, or meetings.
  11. Completion of National Incident Management System/Incident Command System online and face-to-face training courses based on current Public Health Emergency Preparedness requirements.

Environment & Effort:

  1. Depending on program assigned, works inside general office or clinic environments, or works in home environments with potential exposure to dust, dirt, odors, poor ventilation and uncomfortable temperatures. Exposures in home environment could include environmental hazards (lead, mold, animals or vectors), or unsafe living conditions (abuse, drugs, meth labs or weapons).
  2. Exposure Control – Category A. At higher risk for occupational percutaneous or mucosal exposure to blood or body fluids. Per OSHA’s Bloodborne Pathogens standard requirements, CMDHD protects the employee by an established exposure control plan which includes but is not limited to use of universal precautions, engineering controls, work practice controls, personal protective equipment, and worker training.
  3. Some exposure to adverse weather conditions when traveling to other locations.
  4. Physical effort necessary to move about office, clinic and/or home environments; lean and bend to administer treatments or do close inspections; and occasionally lift or move objects weighing up to 25 pounds.
  5. Manual dexterity and feeling by touch necessary to pick, pinch, and palpate; and grasp and use small instruments.

Visual effort regularly required in reading and reviewing records and reports; close inspection or examination; preparing and analyzing data; viewing computer screens; reading scales, thermometers, and gauges; and performing close contact procedures such as drawing blood, dispensing medications, or administering vaccines.
