Education and Experience:

  • Must have one of the following: Bachelor's degree in social, behavior or human services field from an accredited university; or Associate degree in social, behavioral, human service or health-related field; or High School diploma/GED with 2 years of paid or unpaid experience with individuals with Intellectual Developmental Disabilites
  • Must be able to qualify as a Qualified Intellectual Disabilities Professional (QIDP)
  • Preferred to have at least one (1) year of experience in working with individuals with Intellectual and/or Developmental Disabilities
  • Must have valid TX driver's license and be insurable

Job Purpose:

  • Performs Service Coordination to individuals such as preventing, and managing crisis situations
  • Performs assessments, monitors, assessing needs, service planning and coordinating and making appropriate referrals

Job Function:

  • Maintains regular contact with individuals served to monitor services and identify needs
  • Service Coordinator assessments will be completed initially, annual, and/or as needed for all individuals served
  • Will identify gaps in services so individual served experiences a continuum of care to the greatest extent possible
  • Develops Initial and Annual Person Directed Plan and makes revision based on the needs of the individual
