Job Title: Lead Case Manager Division/Program: Youth FSP (Outpatient/Community Based) Starting Compensation: 24.00 - 30.00 USD Per Hour Working Location: Fresno/Clovis, CA, surrounding locations Working Hours/Shift: Monday-Friday 8AM-5PM Why Join Our Team? Employee Recognition & Rewards: A culture that celebrates and rewards your hard work and dedication What you bring to SBHG: Associate degree in a social service field plus two (2) years of experience working in a mental/behavioral health field required (Internship may be substituted for a portion of the work experience) OR Bachelor’s degree in a social science field plus six (6) months of experience preferred. (Internship may be substituted for a portion of the work experience) Valid California Driver’s License required How you will make a difference: Lead and oversee the coordination of intensive case management services for BHC participants, ensure timely assessments, care planning and service delivery. Serve as a primary point of contact with court personnel, including judges, attorneys, and probation officers, and represent the program in court proceedings. Provide clinical and administrative guidance to case managers and support staff assigned to BHC cases. Facilitate and participate in multidisciplinary team meetings, case conferences, and court staffing to monitor progress and address barriers. Collaborate with program leadership on program development, fidelity monitoring, and quality improvement initiatives related to BHC services. Provides services at the TAY Center, in the community, and in TAY’s homes. Uses personal vehicle to drive within the community to provide services and to transport TAY and/or TAY’s family members in personal vehicle. Provides case management services to clients and their families, as deemed necessary and appropriate through the assessment process and in collaboration with the treatment team. Participates in meetings, such as Treatment Team Meetings, to ensure clients’ needs are met and recommendations are followed through. Documents all services provided clearly and concisely within the Electronic Medical Record within the timelines and framework established by Stars. .
