LVN SUPERVISOR
Reporting to the Nurse Manager and Registered Nurse, the Licensed Vocational Nurse acts as the nurse in charge of the Centro San Vicente Wellness Center at Emergence Health Network. The position serves as the immediate on-site supervisor of the nursing staff. Performs basic nursing functions within the clinical setting as well as managing patient flow of all areas. In addition, is responsible for making the weekly schedules and making sure there is adequate coverage for the provider.
Essential Functions:
- Performs Triage and assessment of patients who come to the clinic. Uses nursing judgment and communicates patient’s needs to Provider.
- Serves as a Care Coordinator for Centro San Vicente and Emergence Health Network.
- Performs patient assessment and intake of patients with appointments-following the established procedure for patient care during the intake procedure.
- Works with the nursing staff, the CMO and Providers to provide nursing coverage of the clinical area, which would include assignments for day coverage, evening coverage and any emergency coverage needed.
- Assists providers in the provision of patient care which includes all exams, patient teaching, discharge instructions, referrals, calling patient and locating pertinent lab, x-ray and other diagnostic reports.
- Maintain documentation of all incidents in the clinic; document in the medical record all pertinent information according to the established standard of care. All documentation must be legible.
- Assist in registration and check in process.
- Administer immunizations.
- Supervises the Medical Assistant assigned to the clinical area.
- Provides clinical support coverage.
- Assists in promoting the clinic site.
- Attends clinical/administrative meetings convened by Emergence Health Network, as a representative of Centro San Vicente.
- Responsible for time sheets and payroll.
- Responsible for keeping over-time to a minimum.
- Identify problems or need for assistance and respond accordingly.
- Ability to perform job with minimal supervision and prioritize patient care issues and assigned projects.
- Professional appearance and demeanor.
- Culturally sensitive to the population served at Centro San Vicente and Emergence Health Network.
- Computer knowledge in extracting reports from EMR, Time clock and Microsoft Office.
- Maintains current licensure and certifications.
- Collaborates with management and Senior Leadership.
- Mentors and teaches others.
- Review and revise Policy and Procedures.
- Handles patient complaints and inquiries.
- Assists patients with questions regarding medication use, side effects, etc.
- Ensures the performance of daily Quality Assurance on clinical equipment.
- Communicates and demonstrates the mission, vision and goals of the Center.
- Performs other related duties as assigned..
Minimum Qualifications:
- High School Diploma/GED.
- Current LVN license from the State of Texas and CPR card.
- Two years’ experience in nursing and one year experience in a supervisory role.
- Experience in working with mental health and the homeless population preferred.
- Current Immunization record.
- Ability to work flexible hours.
- Bilingual English/Spanish preferred.
