Pay Rate $25 per hour
Centurion is proud to be the provider of comprehensive healthcare services Indiana Department of Correction. We have partnered with Sondhi Solutions as our subcontractor for psychiatric provider and re-entry services.
We are seeking a part-time (10 hours a week) Transitional Healthcare Facilitator for needs at Plainfield Correctional Facility located in Plainfield, Indiana.
This part time Transitional Healthcare Facilitator will be responsible for the development and implementation of transitional healthcare plans for incarcerated individuals scheduled for release. The Transitional Healthcare Facilitator will coordinate with the Indiana Department of Correction and other state and local agencies to ensure that pre-discharge planning and preparation is accomplished, and those re-entering society are provided with necessary services and resources.
Qualifications
• Bachelor’s degree in psychology, social work, counseling or related field from an accredited institution required
• Two (2) years of experience working in a correctional setting
• Experience with/knowledge of community and state programs and services
• Two (2) years of experience in re-entry services or release planning preferred
• Must show/obtain current Purified Protein Derivative (PPD) documentation for tuberculosis screening
• Must be appropriately and actively certified in Cardio-Pulmonary Resuscitation (CPR) and Automated External Defibrillator (AED) use
• Must be able to pass background investigation and obtain agency security clearance where applicable
Part Time Hours are 8 hours on Monday and 2 hours flexible though out the week.
