JOB TITLE: Emergency Shelter Case Manager
OVERVIEW: Full-Time, $16.25-$18.00(Depending on Education and Experience), Benefit Package
PRIMARY WORKSITE: Auburn, NY
Position Overview: With the leadership support of the Program Supervisor, Operations Director and Executive Director, the Case Manager is part of a team that ensures the advocacy, assistance, support, placement, and follow up of homeless individuals located within Cayuga County, NY. This assistance and support include intakes, reports, client support system meetings, and housing searches among other duties. This position is vital to the effective operation of Chapel Houses homeless persons services. This position is also cross trained to assist agency wide, through various Case Management programs and Emergency Shelters.
Responsibilities:
Case Management:
§ Provide comprehensive intake, assessment, and evaluation of prospective clients.
§ Initiate and maintain regular contact with assigned clients, conduct home visits of significant duration to assist them with acquiring skills to meet their goals of self-sufficiency.
§ Maintain statistical accountability and accurate written information for complete case records and provide ongoing assessments and case recommendation.
§ Compile data and complete reports.
§ Ability to access the agencies HMIS and input data, case notes, incidents, etc.
- Maintain accurate documentation including clear and concise communication to staff, HMIS documentation, Logbook entries, and emails to outside agencies.
- Ability to maintain absolute confidentiality regarding clients and agency operations.
- Supervise residents in the program including coaching residents on program policies and rules to maintain a physically and emotionally safe environment for residents and staff. Provide conflict resolution, crisis intervention and supportive counseling as needed.
Client Advocate
- Collaborate with other service providers, family, and other client-identified individuals to arrange for delivery of services and referrals.
- Participate actively in client related case conferences.
- Client advocacy and referrals to supportive services.
Supports Mission of the Agency
- Participate in staff meetings, supervision meetings, professional/in-service training sessions and collaborative meetings as scheduled.
- Maintain connections with outside agencies.
- Ensure professional representation of the agency in the community.
- Other duties as outlined in Position Description, by the Case Manager Supervisor and Agency Executive Director
§ Ability to work courteously with diverse populations, including those receiving / needing mental health services, substance abuse services and recently released from incarceration.
Please note: This position could require the individual to be placed within the rotating schedule of “On-Call”. Flexibility in one’s work schedule to meet the demands of both shelters is required.
Qualifications:
· High school diploma/GED or equivalent required with a minimum of 2 years related experience.
· Associates degree in related field preferred.
· Ability to difuse crisis situations both within team and independently
· Ability to document program events and carry out scheduled or spontaneous tasks.
· Capable of applying all assigned training topics.
· Ability to maintain absolute confidentiality regarding clients and agency operations
· CPR certified or ability / willingness to become.
