LOCATION
This position is based at Wasserman Residence, our post-acute care center, located at 6121 Montrose Road in Rockville, Maryland.
SHIFTS & STATUS
This is an EXEMPT, FULL TIME position on the DAYLIGHT shift with hours from 8AM to 4:30PM.
OUR RN CLINICAL TEAM MANAGER POSITION
The primary focus of the RN Clinical Team Manager position is to supervise the day-to-day nursing activities of the facility in accordance with current federal, state, and local standards, guidelines, and regulations that govern our facility, and to provide the highest degree of quality care and customer service. This position reports to the Director of Nursing.
MAIN RESPONSIBILITIES
- Assists the Director of Nursing Services in directing the day-to-day functions of the Nursing Department.
- Works collaboratively and in coordination with the MDS Manager and PI Manager assigned to their Unit to drive quality and financial performance.
- Evaluates residents’ status and unit activities at the beginning of the shift and assures that appropriate levels of nursing staff are available to provide care. Plans and delegates assignments to nursing staff.
- Supports staff allocation, recommends staffing levels as appropriate. Shows leadership with the unit staff and helps develop staff.
- Makes frequent rounds on units to evaluate resident status and staff performance
- Participate in the development, coordination, maintenance and implementation of the facility’s QAPI program for the nursing unit. Monitor falls with injury, moderate and severe pain, PU’s, UTI’s, catheters, restraints, ADL declines, and other Quality Measures as assigned.
- Participate in facility surveys (inspections) made by authorized government agencies.
- Conducts and leads weekly unit meetings with nursing staff, Performance Improvement Manager, and the MDS Manager. Communicates information and changes in facility policies and procedures to staff.
- Collaborates with the team to assure resident assessments and care plans are developed, reviewed, and revised as necessary and in compliance with regulatory standards.
- Assures that physicians’ orders are current, appropriate, and communicated to unit nursing personnel.
- Assures documentation and investigation of incidents and accidents, monitors for accuracy and completeness.
- Conducts staff performance appraisals. Maintains anecdotal notes and other records pertaining to employee performance.
- Counsels, educates, and coaches’ employees to maintain, improve, and when needed, correct competencies and work habits to support the mission of the home.
- Makes recommendations to nursing administration regarding hiring, transfer, promotion, and termination of nursing department personnel; Participates in recruitment and retention programs.
- Conducts environmental rounds; inspects the environment; identifies and reports environmental conditions that are present and a risk and submits requests for correction or repair.
- Monitors and evaluates infection control practices in collaboration with the facility infection control preventionist.
- Maintains authorized Unit Budgets as it pertains to staffing, overtime supplies, medical supplies, incontinent supplies, minor equipment purchases and mattress rentals. Makes recommendations to the Director of Nursing. Completes budget variance reports monthly and able to justify being over or under budget
- Conducts root cause analysis of quality indicators that deviate from acceptable standards; identifies patterns and trends; develops a plan to achieve acceptable standards.
- Assures that unit practices are consistent with professional nursing standards following HIPAA requirements.
- Collaborates with ancillary services for efficient, timely and accurate delivery of services and reports (e.g., pharmacy, laboratory, therapies, diagnostics
- Provides direct patient care when needed.
- Adheres to professional code of ethics.
- Ensures that Residents Rights are adhered to at all times.
- Recognize unusual or emergency situations that warrant intervention
- Work one off shift every other month to evaluate staff performance. Convey key information to staff, monitors for quality and Standards of Behavior with off shifts
EDUCATION & TRAINING
- Registered Nurse from an accredited college, university or program, and licensure to practice as registered nurse in the State of Maryland or under the board of nursing’s guidelines for compact states. A bachelor’s degree strongly preferred.
- Two years of experience as a charge nurse in a geriatric care setting with progressive experience in administrative and supervisory capabilities.
- Training in rehabilitative and restorative nursing practices.
