Cherishaven Hospice is a locally based hospice agency. We are a startup agency looking for compassionate and knowledgeable Spiritual Counselor to care for patients in Alameda and Contra Costa County.
Spiritual Counselor will be working under the direction of the Director of Patient Care Services, and in collaboration with Interdisciplinary Team members. Spiritual Counselor will assess, diagnose, and treat patient’s spiritual conditions, counsel patients and families, regarding adjustment to life threatening illness and long term disability. Provides information, referral, and consultative services to patients, families and staff.
Essential Job Functions:
- Assures spiritual assessments of patients and families in the hospice program and appropriate service in a timely manner.
- Develop individualized plan of care and participating in th comprehensive assessment to meet identified spiritual needs.
- Maintain records of spiritual care services utilization and related activities for quality assurance, program development.
- Attends Patient care conference and interdisciplinary group meetings as a member.
- Provides consultation, education and support to the interdisciplinary group on spiritual care.
- Accepts and performs other related duties and responsibilities as required.
Qualifications:
- Graduate of accredited seminary school of theology or equivalent.
- Ability to work as a member of interdisciplinary team.
- Comfort patient and family in an interfaith setting.
- Ability to accept different beliefs, cultures and values.
- Commitment to hospice philosophy of care.
Job Types: Part-time, Contract
Pay: $25.00 - $32.00 per hour
