POSITION SUMMARY
The essential function of the position within the organization is to assess client needs and arrange for and monitor the provision of services. The position is responsible for performing psychosocial assessments of clients, monitoring provision of services, developing and implementing comprehensive care plans, coordinating services and making referrals to community agencies, consulting with clients, making in-home visits, researching previous and performing related clerical tasks. The position reports to Program Manager
MAJOR DUTIES AND RESPONSIBILITIES include the following. Other duties may be assigned.
Performs psychosocial assessments of clients by telephone and during in-home visits to determine client needs.
Compiles and enters data into ESP/CHAT and AIMS database; develops and writes care plans for eligible clients.
Coordinates services for clients (makes referrals to community agencies)
Consults with clients, Caregivers, family members, regarding social programs such as
Medicare Part D, Medicaid, legal aid, in-home care options such as Hospice, nursing homes, assisted living, or independent senior living.
Researches resources and network services for clients. Promotes, improvements to clients quality of life.

Performs clerical tasks such as preparing correspondence or email performs data entry, prepares reports, faxing, photocopy and telephone communication.
Attends staff and other professional meetings, attends technical or professional classes, workshops or seminars to improve and enhance professional skills.
Compiles, examines or evaluates data or information provides input, and possibly recommends action.
Uses addition, subtraction, multiplication and division; may compute ratios rates and percentages.
Reads journals and manuals; composes specialized reports and business letters using proper format and grammar; speaks to groups with coworkers and persons in the community.
Performs job responsibilities adhering to ARC/County regulations. Maintains focus and flexibility while performing tasks.
May complete initial assessments as needed and/or situations dictates.
Other duties as assigned.
Education and Experience
Bachelors Degree from accredited four-year college or university in Social Work,
Counseling, Gerontology, Psychology, or related field.
Possess or pursue Care Management certification within 6 months of employment
Certificates, Licenses and Registrations
Valid Georgia driver’s license and a satisfactory motor vehicle record.
CPR and First Aid within six months of hire.
PT Case Manager Senior Services
Knowledge, Skills and Abilities
Knowledge of federal, state, and ARC guidelines regarding case management as well as county policies and procedures.
Knowledge and ability to operate computer system.
Knowledge and ability to complete various state, federal, and departmental forms.
Knowledge of theories and concepts related to social service care plan, planning and implementations for Senior Services programs.
Knowledge of counseling techniques.
Knowledge of community resources available to seniors. .
Ability to effectively communicate with the elderly and other professionals as well as ability to maintain sensitivity and confidentiality; HIPPA Guidelines
Skill in communicating both verbally and in writing, organizing, interviewing, and record keeping case files.
