What You’ll Do
- Develop and nurture relationships with network providers
- Lead efforts to secure and maintain preferred provider partnerships
- Collaborate with healthcare professionals to enhance care transitions
- Coordinate services across multiple care settings to support residents’ needs
- Bachelor’s degree in Healthcare Administration, Social Work, or a related field
- Experience in Long-Term Care, Case Management, Home Care, Hospice, or Transitional Care preferred (related experience considered).
- Strong communication and relationship-building skills.
