Summary
To prevent dissolutions/disruptions, provide families with in-home support services, and strengthen families to prevent out-of-home placement, the Family Permanency Program of the North Country (FPPNC) works exclusively with families who have finalized the adoption of their child(ren), and families with legal guardianship of their child(ren), with or without a KinGAP agreement. The Family Engagement Specialist uses a diverse and strength-based approach for the direct interaction with the youth, Post Adoptive/Guardianship Parents, and the Community. The Family Engagement Specialist assists the Adoptive Parents and Guardians with effective behavior modification techniques and safety planning and links and refers families to community service providers that provide additional support. The FPPNC serves families in Jefferson, Lewis, St. Lawrence, Franklin, and Oswego Counties.
Key Responsibilities
1.Responsible to deliver high intensity case management to assigned youth and families
2.Completes strength-based assessments, works with the family to create goals/treatment plans, and holds/facilitates regular treatment team meetings
3.Identifies areas of concern/barriers to goal achievement, discusses immediate needs and concerns with youth and caregivers, assists in the development of safety/emergency plans
4.To provide regular and ongoing support, conducts required home visits
5.Monitors physical, mental, and behavioral health concerns, obtains regular updates from providers, makes referrals as necessary
6.Performs risk and safety assessments in the family home
7.Complete all New York State/CHJC mandated reports and forms in a timely fashion, maintains case records and prepares all monthly reports, provides thorough documentation of all casework activities
8.Create peer support group activities for youth
9. Assist in recruitment and community education efforts
10.Participates in 24-hour on-call crisis response rotation
Education Requirements
Bachelor’s Degree in a Human Services or related field required, with at least one year of Child Welfare experience. Experience working with youth at risk and families, experience in Child Welfare, and/or experience in Casework Services. Ability to work with culturally diverse families. Understanding of and experience with Strength-Based and Individualized Care Approach to working with children and families. Requires self-direction, tact, diplomacy, and the ability to be clear, courteous, and professional. Must demonstrate effective oral and written communication skills and the ability to work cooperatively with others. Must be detail oriented.
Valid NYS Driver's License Required.
