Job Title: Structured Family Care Nurse (SFC Nurse)
Overview:
CTI-DRS, a leader in healthcare services, is seeking a highly skilled and compassionate nurse to join our team as a Structured Family Care Nurse. The SFC Nurse will be responsible for conducting monthly health and wellness visits, including head to toe assessments, on individuals receiving Structured Family Care services.
The successful candidate will document the assessment and progress notes in Therap Services, our electronic health record system. Compensation for this position includes hourly pay for each visit as well as reimbursement for drive-time starting at $20 / hour.
Responsibilities:
- Conduct monthly head to toe assessments on individuals receiving Structured Family Care services.
- Document the assessment and progress notes in Therap Services.
- Communicate effectively with the treatment team, including the treating physician, and make necessary recommendations for treatment interventions.
- Work collaboratively with other healthcare professionals to ensure the overall well-being of the individual.
- Maintain accurate records in accordance with the guidelines set forth by IAC 455 regulations.
- Adhere to all company policies, procedures, and regulations while providing high-quality care.
Qualifications:
- Current and active nursing license in the state of Indiana.
- Minimum of two years of nursing experience with direct patient care.
- Familiarity with electronic health record systems, specifically Therap Services.
- Excellent communication and interpersonal skills.
- Compassionate and patient-focused approach to care.
- Familiarity with IAC 455 regulations and the ability to meet these standards.
Hours and Compensation:
This is a part-time position with a flexible schedule to accommodate home visits. Hourly compensation for each home visit will be provided in addition to reimbursement for drive-time. CTI-DRS offers competitive compensation packages and benefits.
