As a Home Health Registered Nurse Case Manager, you will play a pivotal role in coordinating and providing comprehensive healthcare services to our patients in their homes. - Conducting thorough patient assessments to develop personalized care plans
- Administering skilled nursing care in accordance with physician's orders and established protocols
- Monitoring and documenting patient progress, ensuring accurate and timely charting
- Educating patients and their families on disease management, treatment plans, and self-care techniques
- Collaborating with physicians, therapists, and other healthcare professionals to coordinate and ensure seamless care transitions
- Participating in case conferences and interdisciplinary team meetings to review patient progress and adjust care plans as needed
- Complying with all regulatory guidelines, including OASIS assessment and documentation
Qualifications:
- Active Registered Nurse (RN) license in the state of [State]
