COMMUNITY HEALTH WORKER

We are hiring a full-time Community Health Worker to assist with care coordination, appointment scheduling, documentation, community outreach, and in-person home/school visits.
Schedule
This position will work 40 hours/week, Monday-Friday on Day Shift. The scheduled hours are flexible as long as we can meet patient needs. No weekends, no holidays, no over-time, no on-call.
Qualifications
To be considered, candidates must have a high school diploma or equivalent, 2-5 years of work experience in a related job discipline, and have a reliable vehicle/valid driver's license. Current Community Health Worker (CHW) certification in the state of Ohio is preferred but not required. Comprehensive job description provided upon request.
PAY RANGE: $18.59-$22.00 per hour
Actual salaries depend on a variety of factors, including experience, specialty, education, and hospital need.
